Medicat Patient Portal Documentation Upload Instructions

1. Go to http://centralmethodist.medicatconnect.com/home.aspx
2. Click “Uploads” on the left-hand side
3. Click “Select File” on the line for “Physical Form”
4. Upload the 2 forms found on the CMU AT Website
   1. Athletic Pre-Participation Physical Evaluation-History Form
   2. Verification of Current Medical Coverage Form
   3. Copy of Insurance Card (Front and Back)
5. Instructions for scanning your physical forms are as follows:
   1. Scan and save your documents individually using scanning software on a smartphone or computer.  Make sure your First and Last Name and Student ID Number are on all documents.
      1. All documents must be saved as PDF (NOT A picture from camera roll)
      2. Files must be smaller than 4 MB and in black and white
      3. Be sure your file names do not include special characters
